FREE READER PROGRAMS FOR DOWNLOAD:

To use these forms you must have the appropriate program installed on your computer.  Free readers can be downloaded from the software manufacturers.  For PDF files you must have Adobe Acrobat or Adobe Acrobat Reader.  For Word and Excel files you must have Microsoft Office or the Word or Excel viewer, respectively.
    RIGHT CLICK on the PDF, Word, or Excel icon to download the form
    LEFT CLICK on the PDF, Word, or Excel icon to read on line

	Download Adobe Acrobat Reader by clicking on the icon to the left if you do not have Adobe Acrobat installed on your computer
	Download Microsoft Word Viewer by clicking on the icon to the left if you do not have Word installed on your computer
	Download Microsoft Excel Viewer by clicking on the icon to the left if you do not have Excel installed on your computer

A note on the WORD documents (does not apply to PDF documents):
1 - If the documents contain gray areas - that is where data is mean to be entered.  You can enter your data there, then use tab to move to the next area.
2 - The forms are locked to prevent accidentally changing the form.  To edit the form itself, left click on the View Menu and left click on “toolbars”, then left click on “forms” to add the forms toolbar.  Then click on the icon in that toolbar at the far right that looks like a padlock - this will unlock the form so you can edit the form itself.

If you are asked for a password, just press CANCEL
